7.2 To request information in terms of PAIA, the following procedure is prescribed:
7.2.1 The prescribed form must be used for the request. The form can be downloaded from the Department of Justice Websites at www.doj.gov.za.
7.2.2 The request must be sent to info@tyrelife.co.za.
7.2.3 Sufficient detail must be provided to enable us to clearly identify the record and your identity.
7.2.4 The request must identify the right that you are seeking to protect or that you wish to exercise and must provide an explanation as to why the record is required for the protection or exercise of the rights.
7.2.5 You must provide positive documentary proof of identification and, if you are making the request on behalf of another person, you must submit proof of the authority to do so.
7.2.6 You shall be liable for payment of a prescribed fee for the search, reproduction and preparation of the requested record.
